Opioid dependence management

Opioid dependent individuals will be recruited for participation in the study. Upon recruitment into the randomized trial each participant will be started on an induction dosage of medication intended to suppress withdrawal (either buprenorphine-naloxone, morphine, or tramadol). Participants will remain on the induction dose for at least 24 hrs. Severity of withdrawal will be assessed twice a day using the Clinical Opiate Withdrawal Scale (COWS) and the Subjective Opiate Withdrawal Scale (SOWS). Each morning, participants will be given the option to progress on the tapering protocol (see below for individual regimens). Time to complete detoxification and initiation of naltrexone treatment will be measured. At completion of the study each participant will complete a 7-iten Detoxification Treatment Satisfaction questionnaire. Taper Schedule Dosing Buprenorphine-Naloxone (Suboxone): Patients will get 4mg when withdrawal symptoms start. Patients can receive the first 8mg dose as earl...

The Iron box principle

Image
When I was young.. we had an iron box at home, which was doing its work so perfectly.. that we were all afraid to handle it. We had to be very careful, to maintain its temperature to the absolute precision, to get our clothes done without burning it. You have to test it on a dummy cloth first, to find if the temperature is just adequate, switch off and then iron for half a minute.. then rewire it for the next half a minute step. If you iron with it, when you are not in your cool or when you are prone to misjudge.. you will certainly land up, looking through your clothes. The intense protocols with the ironbox made us vary.. that we would resort back to our traditional method of folding pour dresses and putting them under our bed at times. Years later.. Mom brought a automatic iron box. The operation mechanism was interesting. It had a set point for every kind of cloth. And amazingly.. it would just cut off itself, and turn to cooling itself, when the temperature exceeded....

YES for a NO

Image
When was the last time you had   NO for a response..   A well defined, clear cut and squared off NO. How we despise having NO for an answer? What all trajectories we follow for a escapade from a NO? Have you thought at some point of time, when you received a straight and clearcut NO, that this person is mean, insensitive, not worth having a relation with? Have you thought, if they had rather delivered a butter coated version of it..it would have been easier to swallow? Have you spent, disturbed days and sleepless nights regretting over a NO? How long did it take accepting a NO? And how did it change you as a person? If a NO is affecting you so badly,   Or it did make you a much unwelcome version of yourself,   making you act out or regress inwards.. then it is time we reconsider your schemas. 1)       We are social beings, with necessity to interact consistently. 2)       Interactions m...
